Records Requests

Under state law, records generated by the UW–Madison Police Department may be subject to release. All open records requests should be made through the UWPD Records Unit.

Use the form below to file a records request online. Try to be as specific as possible in your request. The department processes and releases all records in a reasonable and timely manner, in compliance with the State of Wisconsin’s open records laws. We will notify the requester when the request has been completed.

    *no charges for $5.00 and under

    General Copies

    • Copy per page (black/white): $0.10 per page
    • Copy per page (color): $0.15 per page
    • Police reports: $0.10 per page, including electronic delivery

    Video/Audio/Photos

    • CD or DVD (4.7GB): $2.50 per disk

    Redaction – Audio/Video*

    •  Actual, necessary, and direct cost of redacting

    • 16GB: $6.00
    • 32GB: $8.00
    • 64GB: $12.00
    • 128GB: $26.00

    Requests in excess of $5.00 and requests for photos, video and/or audio may require pre-payment.

    If pre-payment is required, UW-Madison will send you an invoice. Please do not send payment when your request is submitted.

    UWPD prefers all records requests to be submitted via our online form above. However, requests may also be submitted by email at records@uwpd.wisc.edu or by Fax (608) 262-9768.     * In accordance with 2023 Wisconsin Act 253, a law enforcement agency may impose a fee upon a requester for the actual, necessary, and direct cost of redacting recorded audio or video content. Exemptions are granted to individuals who meet the criteria outlined in Act 253.
